2022-2023 Lunch Form

Digital Blue Card
For the health and safety for all BTECH students, it is required for every student to have both a digital card blue card and physical blue card on file. If you have previously completed the form, thank you and please do not resubmit your information. If you have not completed the Digital Blue Card for your student, please use the link. The deadline to have this form completed is Friday, September 30th. 

Military Opt Form
Federal law requires the NYC Department of Education to provide the names, addresses, and telephone numbers of high school students to military recruiters and institutions of higher education that request this information. While we are committed to protecting the confidentiality of our students, we must comply with the law.  Please click the link below to read the form and complete it.

MetroCard Form

In order to obtain a new MetroCard you must sign into your NYC Public Schools account and complete the MetroCard Request Form. You can also stop by Ms. Santana’s Desk in the Main Office. Once you complete the form, allow 48 hours for your request to be completed. You will receive a confirmation email from Ms. Santana that your MetroCard is ready and pick it up from the Main Office during 5th period.
